Does the patient have asymmetrical synovitis?

Does the patient have synovitis that is predominantly in the lower extremities?

Does the patient have a family history of ankylosing spondylitis, psoriasis, acute uveitis, reactive arthritis or inflammatory bowel disease?

Does the patient have psoriasis?

Does the patient have ulcerative colitis or Crohn's disease?

Did the patient have a history of nongonococcal urethritis 1 month prior to the onset of the arthritis?

Did the patient have a history of nongonococcal urethritis or cervicitis 1 month prior to the onset of the arthritis?

Did the patient have a history of acute diarrhea 1 month prior to the onset of the arthrits?

Does the patient have buttock pain that alternates between the left and right gluteal areas?

Does the patient have pain or tenderness at the site of insertion of the Achilles tendon or plantar fascia?
