Is the total volume much larger or smaller than normal?

Did the recipe require much of an additive than usual?

Is there anything unusual about the product's appearance (color, precipitate, etc)?

Has the patient shown any unexpected or unusual response to the product?

Has the product undergone a complete quality check prior to release?

Is the product outdated?

Was the product compounded at night or on the weekends?

Was the product compounded by a new employee?
