Number of close relatives with documented colorectal cancer

Number of close relatives with documented adenocarcinoma of the endometrium and/or ovary

Number of close relatives with documented adenocarcinoma of the stomach and/or small bowel

Number of close relatives with documented adenocarcinoma of the pancreas and/or biliary tract

Number of close relatives with documented transitional cell carcinoma of the renal pelvis and/or ureter

Number of close relatives with documented brain cancer or a sebaceous tumor of the skin

Is at least one of these relatives a first degree relation of another affected relative?

Number of generations involved by colorectal cancer

Age of the youngest patient when a colonic primary was first diagnosed

Has familial adenomatous polyposis been excluded?

Were the tumors confirmed by pathologic examination?
