Does the patient have a chronic cardiovascular disease such as CHF or cardiomyopathy?

Does the patient have chronic pulmonary disease including COPD and emphysema?

Does the patient smoke cigarettes?

Does the patient have diabetes mellitus?

Does the patient have chronic liver disease or cirrhosis?

Is the patient an active alcoholic?

Does the patient have a chronic leakage of cerebrospinal fluid?

Does the patient have a cochlear implant?

Does the patient have sickle cell disease?

Does the patient have anatomic or functional asplenia or significant splenic dysfunction?

Does the patient live in a nursing home or long-term care facility?

Is the patient involved with group child care?

Is the patient an Alaskan native or American Indian at risk for invasive pneumococcal disease?

Is the patient HIV-positive?

Has the patient received an organ or bone marrow transplant?

Is the patient receiving long term corticosteroid therapy?

Is the patient receiving long term immunosuppressive chemotherapy?

Does the patient have chronic renal failure?

Does the patient have the nephrotic syndrome?

Does the patient have a malignancy, including leukemia, lymphoma, Hodgkin's disease or multiple myeloma?

Is the patient receiving chemotherapy or radiation therapy for the malignancy now?
