Injury Severity Score (ISS)

Glasgow Coma Score (enter from 3 to 15)

Was a coagulopathy noted in the operating room?

Was hypotension present in the operating room?

Was there intimal disruption in a femoral artery?

Did the patient have a thoracic injury?

Was there a bony fracture?

Did examination of the lower extremity with the injured femoral vessels show?

• signs of distal ischemia?

• absent or diminished pulses?

• an expanding hematoma?

• a palpable thrill?

• pulsatile bleeding?

• the presence of a bruit?
