• 2 or more febrile episodes of unknown source?

• a febrile episode of unknown source lasting for 2 weeks or more?

• fever for 2 weeks or more while receiving appropriate antibiotic therapy?

• lung rales without fluid overload?

• presence of a nasal eschar, ulcer or discharge with epistaxis?

• sinus tenderness?

• pleuritic chest pain?

Does the chest X-ray show ...

• an infiltrate first seen after hospital day 14?

• a pulmonary infiltrate in 2 or more lobes?

• lung nodules or cavities?
