Extravasation of Drug from Central Intravascular Catheter | Risk for Chemical Mediastinitis

Are you administering a drug by a central intravascular catheter?

Were there any difficulties noted during catheter placement?

Are you concerned that the catheter tip may have migrated?

Is there difficulty in withdrawing blood from the catheter?

Do you suspect that there may be a thrombosis about the catheter?

Are you having increased difficulty infusing fluids through the line?

Has the patient developed an unexplained pleuritic or substernal chest pain?

Is the drug being administered irritating to tissues?

Is the drug concentration in the infusate relatively high?
