American College of Rheumatology (ACR) Criteria for the Diagnosis of Churg-Strauss Syndrome

Does the patient have a history of asthma?

Percent eosinophils in the white blood cell differential (enter a number from 0 to 100)

Does the patient have a mononeuropathy, multiple mononeuropathy or polyneuropathy attributable to a systemic vasculitis?

Does the patient have one or more migratory or transitory pulmonary infiltrates on chest X-ray?

Does the patient have a history of acute or chronic paranasal sinus pain or tenderness?

Does the patient have a history of radiographic opacification of the paranasal sinuses?

Does a biopsy show blood vessels with an accumulation of eosinophils in extravascular areas?
